I am at least 18 years old and I voluntarily consent to receive a tattoo today. I understand the procedure involves risks including, but not limited to, pain, bleeding, infection, scarring or keloids, allergic reactions (e.g., to pigments/soaps), delayed healing, and potential interference with MRIs. I agree to follow all aftercare instructions provided to me.
To the maximum extent permitted by Illinois law, I release and hold harmless INKY tattoo studio, its owners, and the artist from liability for injuries or damages arising from the procedure, except for acts of gross negligence or willful misconduct. Nothing here waives rights that cannot be waived under Illinois law.
For any lettering, symbols, or designs I request, I have verified the spelling/meaning and accept full responsibility for accuracy.
I consent to non-identifying photos/videos of my tattoo for the studio’s portfolio/social media (optional).
